🧾Sea Service Documentation Checklist

Keep this checklist handy for your next gig or USCG application: 📄 Download the Sea Service Documentation Checklist (PDF)

Required Info for Each Vessel:

  1. Vessel Name
  2. Official Number or State Registration
  3. Owner’s Name and Contact Info
  4. Vessel Gross Tonnage (if known)
  5. Type of Vessel (sportfishing, sailboat, tug, etc.)
  6. Type of Operation (private, commercial, for-hire, etc.)
  7. Your Role (deckhand, mate, captain)
  8. Start and End Dates of Service
  9. Avg. Days per Month
  10. Total Days Underway (4+ hours counts as a day)
  11. Waters Operated On (inland, near coastal, offshore)
  12. Vessel Owner Signature (you sign if self-owned)

Additional Notes:

  • 360 days of sea time required in the past 5 years (90 in the last 3)
  • Submit one CG-719S form per vessel
  • Proof of ownership required for self-owned boats
